Join Quokka Care as a Health Coach and play a direct role in keeping patients healthier at home. You'll manage a defined patient population, monitor vital trends, and act as an extension of the provider's office.
The Health Coach role is vital to both the success of our business and the health of the patients we serve. A Quokka Care Health Coach manages a defined patient population, monitors vital trends, documents patient encounters, and provides structured patient outreach. As an extension of the provider's office, delivering an exceptional patient experience is key. This position reports to the Health Coach Manager.
A typical week as a Quokka Care Health Coach includes:
Provide individualized care and build actionable patient care plans.
Monitor patient vital readings and escalate concerns to the patient's provider as indicated.
Conduct regular patient outreach to support health goals.
Document all patient interactions in the Quokka Care platform.
Develop strong relationships with patients and providers.
Promptly respond to communications and questions from patients and providers.
Achieve patient engagement program goals — minimum of one Health Coach / patient phone conversation per month.
Collaborate with our Product team to enhance the Quokka Care platform.
Improve patient outcomes — help patients avoid the need for crisis care through early intervention.
